Requirements:
Documents will only be issued to owner or authorized representative with written authorization. Owner or representative must bring:
a) Original Licence issued by UPKJ;
b) ORIGINAL Unit Pendaftaran Kontraktor (UPKJ) Certificate and ORIGINAL PERAKUAN KONTRAKTOR (PPK-CIDB) Certificate - where applicable.

Submission:
Completed documents in envelope marked with Tender No. and Tender to be deposited into the Tender Box and submitted to:
